Hi

Do you mean a full scale new island; with roads and trees and lakes and forrests houses and towns?

Or do you just mean a missions like Domination or CTI or Berzerk or a new Hold mission, or your own coop mission?

If it is the latter ArmA II includes an inbuilt mission editor.

Other wise to make whole islands you need the BI Editing Tool Suite as linked above.
