Good controls to put on the 2 extra mouse buttons

I have 2 extra mouse buttons on the side of my mouse that are unused to my knowledge in Arma 2.

What commands do you suggest I should bind to them?

i use the bottom button for teamspeak and the top button for ingame chat.
but i am sure every player has his/her own favorite

One for "looking around" freeing head, and one for VOIP/TS. Without TrackIR so far i find it most intuitive to have the freelook there.

I would suggest "switch weapon mode" or whatever it's called (ROF/Grenade switching). The other button I have is for centering trackIR. I would suggest that whatever you choose, to also separate the zoom function and ironsight mode. I personally use right mouse for zoom and V for iron sights. I think it's right mouse for both by default IIRC.

I would say, hold breath one button, and jump obstacle the other.

I use one for optics and turn it off on right mouse button. Therefore the right mouse button is for zooming in and my extra button near thumb is for optics.. Been doing that since arma1 and it has worked great.

I never really liked the one click right mouse button for optics and then holding it down to zoom.
